GUIDE

Does the fat come back after fat freezing?

The fat cells that a cycle clears do not come back. A share of the fat cells cooled to the treatment temperature die and are carried away by the body over the following weeks, and the body does not replace them. What can change is the size of the cells left behind: they can swell with weight gain, in the treated area as everywhere else. So the honest answer is that the result holds while your weight does, and that is what the one long follow-up, of two patients, shows.

What the follow-ups show

  • A 2016 report follows two patients photographed six and nine years after treatment of one flank, the other left untreated for comparison, with the reduction still visible.
  • The manufacturer's flank study followed 60 patients to six months and measured the fat layer 18.7% thinner on ultrasound.
  • A review of 518 patients found the fat layer 23% thinner measured with skin callipers at three months.

The manufacturer describes the cleared cells as eliminated, and says the remaining cells can enlarge. Both halves of that are what we see at reviews.

What makes it look as if it came back

  • Weight gain of a few kilos. The treated area grows with the rest of you, though with fewer cells than before. Lose the weight and it recedes again.
  • A second bulge next to the first. Treating the lower abdomen does not treat the upper abdomen; what you notice afterwards may be the untreated neighbour.
  • The rare opposite. In about 1 in 3,000 treatments by the manufacturer's figure, the treated fat grows larger and firmer instead of smaller, two to five months after the cycle. A 2025 analysis that combined 28 studies put it at 0.22% of patients. It is called paradoxical adipose hyperplasia, it does not go away on its own, and surgery may be needed to correct it. The downside page explains it.

Keeping the result

Nothing special: the weight you had on the day of treatment, kept. People who want the area firmer as well as smaller combine it with EMsculpt NEO at the same clinic, which builds muscle under the fat, and the full guide compares the two.
