CoolSculpting is FDA-cleared in the US for the thighs since 2014, inner and outer, in people with a body mass index of 30 or under. The inner thigh had the highest satisfaction in the manufacturer's whole study table: 45 patients, one treatment, 93.3% satisfied at four months, with the fat layer 2.8 mm thinner and independent doctors picking out the before photograph correctly in 90.5% of cases. The outer thigh was studied in 40 patients: independent doctors picked out the before photograph correctly in 83.9% of cases, the layer 2.5 mm thinner, 86.5% satisfied. Both thighs are treated in one visit, two to four cycles per area, usually with no time off work, at Cool Clinic, 130 Harley Street, with the price in writing at a free consultation.
Inner and outer
The inner thigh is a soft pad that the applicator draws in; the outer thigh is a firmer, flatter bulge. The two are quoted as separate areas, and both sides of each are treated in the same visit because two applicators run at once.
What the studies measured

- Outer thigh, 40 patients: one treatment, independent doctors picked out the before photograph correctly in 83.9% of cases at four months, the layer 2.5 mm thinner, 86.5% satisfied.
- Cycles: two to four per area by the 2022 expert panel, with four to eight weeks between treatments; the panel notes the thighs may need a longer gap between treatments.
The session and after
Two cycles at a time, one on each thigh, 35 to 60 minutes, intense cold and pulling for the first minutes and numbness after. The thighs are red, firm and often bruised for a week or two and numb for one to three weeks; in our experience you can walk and work the same day. The result can start to show from about four weeks and is judged about eight weeks after the last cycle, when we take the after photograph.
Who it suits
A pinchable inner thigh or a clear outer-thigh bulge in someone whose weight is steady. Loose skin on the inner thigh after weight loss does not tighten with cold, and the clinician says so at the free consultation.
